This document sets out the Terms & Conditions of the Lorna Jane Active Elite Program provided by Lorna Jane USA Inc. These Terms & Conditions are intended to protect you and Lorna Jane USA Inc.; and are applicable to you (the Member) from the time of membership card issue to the time of membership expiration or termination.  

Any failure or delay by Lorna Jane in exercising its rights under any of these Terms and Conditions does not constitute a waiver of those rights by Lorna Jane. Any waiver by Lorna Jane must be in writing and signed by an authorized officer of Lorna Jane. QUALIFICATION

The Lorna Jane Active Elite program is exclusively available to fitness professionals engaged in active employment within the fitness industry as an instructor within the United States.

Proof of this employment must be verified prior to acceptance by providing Lorna Jane the below details through the provided form at www.lornajane.com/joinactiveelite

If employed by a business, one the below must be provided:

  • Dated letter from employer on business letter head stating employment
  • Copy of a printed current business card
  • Employers details for phone reference

If self-employed, the below must be provided:

  • Website
  • Copy of a printed current business card

Lorna Jane reserves the right to request additional information or refuse acceptance in to the Active Elite program should the information/ documentation provided not be deemed sufficient to prove active employment in the fitness industry as an instructor within the United States.

 

PROGRAM ACCEPTANCE

After submitting the required verification details, allow up to 5 business days for confirmation of your membership status, sent electronically to the email provided in your application.

If accepted in to the program, your nominated store will contact you within 2 business days to confirm details and extend any private offers if currently available.

Welcome packs (including your Active Elite loyalty card) are dispatched to the provided address within 2 business days of acceptance and may take up 10 business days to arrive.

Active Elite benefits may be redeemed prior to receiving the pack if the welcome email instructions are followed.

 

MEMBERSHIP BENEFITS

The benefits applicable to a member of the Active Elite Program are as outlined below and only available to that member during membership validity. To redeem an Active Elite member specific promotion in a Lorna Jane retail store, the Lorna Jane Active Elite membership card must be presented; and to redeem an Active Elite member specific promotion online the provided code must be entered at cart prior to checking out and the account associated with the membership must be logged in to.

Should proof of membership not be presented at time of purchase in store Lorna Jane will not honor any Active Elite membership benefits or promotion in that transaction. Equally, if proof of membership is not provided and full price is charged for product, Lorna Jane will not credit or refund any difference once the transaction has been processed. Please contact your nominated retail store with any queries surrounding this.

Additional exclusive offers may be communicated by mail, email, SMS, Facebook or in a Lorna Jane retail store. Terms and Conditions to such promotions should include details including, but not limited to, purchase requirements, further eligibility requirements and time frame of promotion validity.

LOST OR STOLEN ACTIVE ELITE CARDS

You must notify the Active Elite Customer Service Team immediately if your Active Elite Program membership card is lost or stolen by contacting 1800-469-3897 customerservice@lornajane.com or the below postal address.

Lorna Jane USA Inc is not liable for any delay in replacement or for any unauthorized use of a Lorna Jane Active Elite Program membership card.

 

TERMINATION

Lorna Jane reserves the right to suspend or terminate your membership in the Lorna Jane Active Elite Program immediately if Lorna Jane reasonably believes that you have breached these Terms and Conditions.

Should you wish to terminate your membership to the Active Elite Program, Lorna Jane requires formal written communication containing the below information. Upon receipt of communication, Lorna Jane will terminate your membership to the Active Elite program immediately.

LORNA JANE ACTIVE ELITE PRIVACY CARD NOTICE

The following contains important privacy information for the Lorna Jane Active Elite program member. This privacy notice is provided to you by Lorna Jane Pty Ltd and contains important information regarding the collection, use, disclosure, security and access of your personal information.

 

COLLECTION OF PERSONAL INFORMATION

The information Lorna Jane collect includes:  

  • Contact Information provided by you to Lorna Jane.
  • Transactional information when you shop using your Lorna Jane Active Elite Card such as, but not limited to, the store(s) you shopped at, when you shopped, your purchases and method of payment.
  • Information that we may gain from other sources relevant to the Lorna Jane Active Elite Card Information you may provide from time to time through contact with the Lorna Jane Support Centre or information you provide when using the on the Lorna Jane website.

 

USE, DISCLOSURE AND PROTECTION

Your personal information will be protected through confidentiality agreements and other contractual obligations where disclosed.  Lorna Jane collects this information and may use this to keep you informed about special promotions, events, product launches and other special offers and discounts, relevant to the Lorna Jane. Lorna Jane will take all reasonable steps to ensure your personal information is protected from unauthorized access, misuse, modification and disclosure while stored by Lorna Jane USA Inc. Lorna Jane may also use your information for research, marketing and product and service development.

It is the Active Elite member’s responsibility to ensure Lorna Jane USA Inc. is provided with correct and updated personal information to receive full benefits of the program.
